中国是一个具有几千年封建传统的国家，不孝有三，无后为大的传统观念以及养儿防老、男尊女卑的陈规陋习根深蒂固。实行计划生育，就是对传统的挑战、对愚昧与无知的挑战，需要千千万万计生工作者的无私奉献和默默付出。

记得那是一个春光明媚、鸟语花香的日子。XX县安福镇计生办热闹非凡，我和其他专干又像往常一样齐聚一堂，参加例会。就在例会开始前，一位年过6旬的女人在王镇长的陪同下泪流满面，她就是文塘村计生专干辛淑元。那一天对于她来说是一个不同寻常的日子，是她人生旅途的一个驿站，她将从计生专干的岗位上退下来，并把计生专干的担子交给自己亲自选定的接班人肖梅香的肩上。

很早我就了解这位可敬的女人，她从事村级计生工作已经整整35年，岁月的风尘洗去了她的青春年华，她也由姑娘变成了媳妇，又从媳妇变成了婆婆。她经历了上个世纪计划生育工作的全过程：70年代的激情演绎，80年代的轰轰烈烈，90年代的和风细雨。无论哪一时期，村级计生专干都是一个至关重要的角色。辛淑元出生清苦，没有上过一天学堂，为了胜任自己的工作，她以超人的毅力巧记硬背，将全村男男女女、老老少少的生辰八字烂熟于心，创造出独一无二的“辛氏台账”。为了攻下一个个计生堡垒，她甚至当卧底，搞跟踪，上演了一幕又一幕斗智斗勇的人生喜剧。她心直口快又乐善好施，处事果断却热心周到，总是用自己的人格魅力影响着身边的人，因此，她不怒自威。

然而，长江后浪推前浪，江山辈有人才出。辛淑元知道自己尽管兢兢业业，毕竟岁月不饶人，已近70岁的她工作起来已经不像年轻时那样挥洒自如了，再说计划生育工作随着重心下移以及优质服务的不断深入，对村专干的要求也越来越高。为了不拖计划生育工作的后腿，辛淑元不得不做出一个痛苦的决定：退任。她开始默默物色接班人，千挑万选终于选中了年轻、热情、文化素质较高的育龄妇女肖梅香。专干例会上，辛淑元动情地说：从事村级计生工作很苦，很累，很得罪人。但我从不后悔。今天，我就要从计生专干的岗位上退下来了，我的心情既高兴又难过。35年来，我与计划生育工作已经结下了不解之缘，以至在独生儿子婚配时，医生、护士、教师我都不爱，单单挑了一个计生人做儿媳，以此延续我的计生梦。今天虽然退任了，但我还要做村里的计生顾问，不要一分钱的回报。

多么朴实的话语，多么崇高的境界。

今天是第19个世界人口日，中国的宣传主题是“生殖健康是一种权利，让我们将它变成现实”。能在天津举办中国“7.11”世界人口日大会，我们感到非常高兴。我谨代表XX市人民政府向国家人口计生委领导、联合国人口基金会驻华代表处官员及各位来宾表示热烈欢迎!

天津是中国四大直辖市之一，是北方最大的沿海开放城市。在经济快速发展的同时，天津各项社会事业也不断取得新的成就。近年来，天津的社会综合发展指标和人类发展指数(hdi)一直位居全国前列。天津是我国最早推行计划生育的城市之一，与人口发展紧密相关的教育、卫生、妇女儿童等事业得到了较快发展。2007年我市人口出生率为7.91‰，继续保持稳定的低生育水平，人口总抚养比为26.76%，形成了良好的人口年龄结构。高等教育毛入学率达到55%。新增劳动者平均受教育年限超过14年。孕产妇死亡率为8.61/10万，婴儿死亡率为5.33‰，人口预期寿命已达到79.39岁。

1994年国际人口和发展大会以后，XX市人民政府依据以人为本的理念，秉承中国政府对国际人发大会行动纲领的承诺，在国家人口计生委的指导下，按照“到2015年世界各国政府要能够向全民提供全系列、高质量的，包括计划生育和性健康在内的生殖健康服务”的要求，市、区(县)各级政府更加重视人口与可持续发展，更加尊重公民的生殖健康权利与生殖健康服务的获得，更加关心妇女和儿童的健康。我们采取多种措施，全力构建和完善从青春期到育龄期、育儿期、孕产期和中老年期的全人口、全过程、全生命周期的生殖健康传播、教育与指导、咨询与服务;从加强三级预防，控制和减少出生缺陷，提高出生人口素质，到在全市范围内全面启动旨在进一步改善妇女儿童健康状况的《XX市妇女儿童健康行动计划》，努力为天津包括外来和流动人口在内的所有育龄人群建立生育关怀的工作体系。XX市人民政府承诺并努力工作的目标是：对每位女性公民一生的权益实行全程维护，终身保障。

首先，推动生殖健康权利和服务的实现，我们注重倡导和传播，加强指导和帮助，使计划生育/生殖健康逐步成为群众乐意接受的自愿选择。

XX市自1998年起至今，广泛深入持久地开展了“婚育新风进万家”活动，目前已进入第三周期。这一活动的目标是促进市民了解国家的计划生育政策，倡导市民建立“计划生育、少生优生、生男生女一样好、男性参与计划生育、让计划生育/生殖健康成为一种文化”的婚育新观念、新风尚和新习俗，帮助市民掌握避孕节育、生殖健康包括性健康的方法，为保障公民的生殖健康权利和获得生殖健康服务发挥了重要作用。我们始终强调开展这一活动要在“进万家”上下功夫，让这一活动深入社区，深入家庭，深入人心，充分发挥人民群众的主体作用，调动起一大批热心婚育文明建设志愿者的积极性。目前全市各社区建立了80多个婚育文明建设群众社团，3000多名社区志愿者骨干活跃在基层群众之中，成为婚育文明的传播者，使“婚育新风进万家”这一群众性精神文明创建活动品牌具有强大的群众基础和旺盛的生命力。

随着形势的发展，“婚育新风进万家”活动也在不断创新，各个社区群众性的社团活动逐步涵盖了统筹解决人口问题所包括的人口数量、素质、结构和迁移等各个方面，形成了丰富多彩、多种多样有天津特色的实现生殖健康权利和服务的实践。有对社会和社区展开生殖健康倡导的，如XX区的“生育文化研究会”传播婚育新风和XX区的“新型生育文化传播工程”;有对育龄妇女展开生殖健康服务的，如XX区的“大手拉小手母亲沙龙”;有对家庭展开健康服务的，如XX区的“家庭计划指导”;有对青少年展开服务的，如XX区的“青少年素质教育指导”;有对流动人口展开生殖健康服务的，如XX区的“流动人口客嫂驿站”;各区(县)还有“社区养老论坛”、“空巢家庭联谊会”、“新型生育文化快乐家苑”、“婚育文明明星计划”、“独生子女母亲教育园地”、“亲情关怀成长驿站”等各种活动，直接面向社区，覆盖妇女、青少年、流动人口和老年人口等不同人群。通过政府的公共服务和非政府组织与志愿者的服务，满足着人民群众日益增长的计划生育/生殖健康需求。

第二，推动生殖健康权利和服务的实现，我们注重提供优质的生殖健康指导与服务，使“人人享有生殖健康服务”的目标逐渐成为现实。

自2001起，XX市政府连续六年将全市生殖健康服务体系建设列入市政府改善人民生活20件实事之中。陆续对18个区县生殖健康服务中心和126个乡镇生殖健康分中心进行了规范化的改扩建，市级生殖健康服务中心正在建设中，这些生殖健康服务网络的建设和服务能力的提升，将大大增加老百姓生殖健康服务的可获得性，增强服务效果。

各级生殖健康服务中心,根据市民需求,开展了多种形式的服务活动。市区以社区居民和与日俱增的外来务工人员为服务主体人群，广泛开展健康助困、健康助残、健康助弱、健康助优和健康指导进社区的“四助一进”及流动人口生殖健康服务活动。农村则以已婚育龄群众为主要服务对象，实施计划生育生殖健康促进计划，全面实现了避孕节育知情选择和基本的生殖健康服务。各服务中心围绕青春期、婚育期、更年期等不同时期遇到的问题开展生育关怀活动，开设了新婚学校、孕妇学校、开展了儿童早期教育、更年期心理干预、男性健康教育等项目，增加了产后康复训练、儿童统合训练、中老年健康指导等服务内容。为倡导安全的性行为，我们改善避孕工具发放方式，推行“一证通”、“一卡通”等新型手段，在社区、机关、单位、大学等设置安全套发放装置，使群众更加容易地获得避孕工具。

有了良好的网络和服务能力，自2001年以来全市接受生殖健康服务的人次以每年20%的速度递增，至2007年已达到502.1万人次，成为XX市公共健康服务的重要组成部分。

第三，推动生殖健康权利和服务的实现，我们注重预防为主，努力控制和降低出生缺陷发生率，提高出生人口素质，使生殖健康和生育质量得到有效保障。

为了进一步提高XX市的生殖健康水平，2006年，XX市政府组织实施了《三级预防减少传染病母婴传播，控制出生缺陷实施方案》，通过孕前、孕中和产后三级干预，对乙肝、艾滋病、梅毒、神经管畸形等9种传染性和遗传性疾病进行筛查，对新生儿甲状腺功能低下、苯丙酮尿症、先天性心脏病和新生儿听力障碍等疾病实施救助。通过实施“早期筛查—及时诊断—适时干预—结果追访”的预防保健工作模式，全市组织对目标人群的筛查达100多万人次，检出阳性病例8000多人次，并进行了积极的干预，造福了众多妇女儿童和家庭。

第四，推动生殖健康权利和服务的实现，我们注重保护妇女儿童的健康，使妇女和儿童健康状况得到进一步改善和提高。

在实现生殖健康权利和服务过程中，我们将切实维护妇女儿童权益，保障妇女儿童健康，保障母婴安全与提高出生人口素质和提高妇女儿童健康水平结合起来，市政府组织实施了2008年—2012年《XX市妇女儿童健康行动计划》。这个行动计划旨在建立保障妇女儿童健康的三个公共卫生服务体系：降低孕产妇、婴幼儿死亡率，保障母婴安全;控制和减少出生缺陷，提高出生人口素质;解决妇女儿童健康的突出问题，改善妇女儿童健康状况。行动计划充分体现了“政府主导，多部门合作，广大妇女儿童参与并受益”的工作思路。今年5月，这个行动计划已在我市正式启动。

XX市计划生育协会和美国适宜卫生组织合作开展“青少年青春健康生活技能培训”项目，得到市、区两级政府大力支持。全市共有80多万青少年以参与互动等生动活泼的方式接受了关于性与生殖健康、预防艾滋病、远离毒品、积极的价值观、健康的人际关系以及人生规划等教育。接受教育者既包括大中小学生，也包括外来务工人员、青年军人乃至服刑人员等。青少年青春健康教育越来越得到各级政府和全社会的高度重视，因为，谁关注青少年，谁就在关注未来;谁赢得了青少年，谁就赢得了未来。
