（4）主要措施

措施，即实现目标的具体活动安排。措施要具体；要符合学生的年龄特征和心理特点，要生动活泼、形式多样，为学生所喜闻乐见；要注重教育效果，不搞形式主义，不做表面文章。措施定了就要执行，不能执行的就不要写进计划。

（5）时间安排

第三层次：计划制订人姓名与制订日期。依次分行写在正文下方。
