沭阳夏商时属徐州，周时属青州，春秋末期为鲁国南境，战国时属楚国，秦初属薛郡，后属郯。西汉置厚丘县，隶属东海郡，立阴平、建陵两侯国。三国时立东海国，领厚丘、阴平、建陵等11县。

南朝宋文帝元嘉四年，并入襄贲，另置僮县。 梁武帝天监五年，废僮县，置僮阳郡。东魏孝静帝武定七年（549）改僮阳郡为沭阳郡。

北周建德七年（578）始定沭阳县名。
