请假条格式

请假条是学生常用的应用文，在教学中，我首先认真钻研教材，制定出教学环节为 ：

一、为什么要写请假条?

二、怎么写?(格式)

三、实践练写

针对第一环节，我先让学生读练习中的第一、二段话，展开讨论，得出：有病或有事不能来校学习或参加某种活动，写好请假条给家长或同学代给老师

如何写呢?让学生读书中的例文，得出：居中写“请假条”三个字;下一行顶格写称呼冒号;另起一行空两格写正文(正文写了几句话，每句话交代什么?);下一行空两格写“此致”另起一行写“敬礼!”，(此处可让学生连起来记忆);最后注名自己的身份和姓名、日期

学生边看例文边总结，然后给学生一定时间消化，将例文完整的看一遍，在脑中留下请假条的正确格式。

第三环节，创设情境：清明节到了，王红要和家人一起去乡下扫墓，星期一一天不能来上课，请你替王红给张老师写一张请假条。学生写时，我注意行间巡视，发现问题及时指正，不能让错误的格式在学生的脑中留下印象。然后，选读部分学生的请假条，看谁正文语言简洁、说得清楚。

四、巩固阶段

在家庭作业或小练笔中在创设情境，提供练习的机会：体育达标运动会要开始了，李明在练掷实心球时不小心将手腕扭伤，医生建议不能做剧烈运动，达标运动会不能参加了，请你替李明写一张请假条给体育王老师。这一练笔，并不用说请假几天，而是说不能参加运动会，跟上次的不一样，提醒学生不可机械记忆，应视具体情况而定正文内容。

五、在学生牢固掌握了正确的请假条格式以后，再把他与留言条格式进行比较，这一环节切不可太早，否则会起误导作用，两种格式会混淆。

“请假条”相当于公文中的“请示”，但比请示简便、灵活，格式可以不固定，也可以固定。一般最常用的请假条格式如下：

标题(居中)：请假条

上款(顶格写部门的名称或领导人的名字)：

正文(请假缘由、起止日期及天数)如：因......需要请假，请假时间自年x月x日至年x月x日共xx天，恳请领导批准。

下款(标在右下)...............请假人：x x x

假条的格式由标题、称呼、正文、落款等部分组成，具体写作方法如下：

标题：“请假条”三字写在第一行中间位置。

称呼：在第二行顶格写明向谁请假。写给收条人的称呼同一般书信。一般都是向顶头上司或老师请假。具体写法是在姓后面加上职务，如“陈主任”，“王科长”，“张老师”等，后用冒号。

正文：写清请假的缘由，请假的起止时间。在第三行空两格写起。最后常以“请予准假”，“此致敬礼”，“不胜感激”等用语结束。

落款：署名和日期的写法同一般书信。在右下方署上请假人姓名。姓名下方标注请假日期。

要写好请假条.必须注意以下几点：①请假原因要具体清楚，不能含糊其辞，模棱两可。②如果遇到突发事情来不及请假，可请人将请假条捎给收条人。如果是事先知道的事情应提前向有关人员请假，得到批准方可。一般请假人不应起级请假。③请假时间一定要写清楚，便于有关人员考虑和安排。④请假人在假期满后应主动销假，假条不必收回。
